Responsibilities:
Provides case management as described in state and federal regulations.
Performs initial and ongoing patient assessments in response to patients' needs and in accordance with physician orders.
Initiates a written plan of care, re-evaluates and updates the plan as necessary.
Re-evaluates patient's nursing needs on a regular basis.
Furnishes services requiring substantial and specialized nursing skill.
Participates in in-service programs, trains, and teaches other nursing personnel.
Assumes responsibility for each patient receiving nursing or personal care.
Develops and implements the nursing care for the patient under the direction of the physician.
Provides nursing services, treatments and diagnostic procedures requiring specialized skills.
Documents skilled care and/or services provided in compliance with federal and state laws and regulations, and agency policies and procedures.
Observes and reports symptoms, reaction to treatments, drugs, and changes in the patient' s physical or emotional condition and reports to physician and Director of Nursing.
Initiates preventative and rehabilitative nursing procedures as appropriate for the patient' s care and safety.
Responsible for clinical and progress notes for each patient receiving care.
Coordinates services as needed for the patient.
Requirements:
A RN with a bachelor' s degree in nursing with 1 year of experience in an active treatment unit in a psychiatric or mental health hospital or outpatient clinic.
A RN with a diploma or associate' s degree with 2 years experience in an active treatment unit in a psychiatric or mental health hospital or outpatient clinic.
On an individual basis, other combinations of education and experience may be considered.
